Posteriorly, the left lung is rendered with emphasis on the inferior basal vein draining the posteroinferior (basal) segments of the left lower lobe and converging toward the inferior pulmonary vein at the hilum. Segmental bronchi to the lower lobe course nearby, with the venous branches typically running intersegmentally and lying anterior to their corresponding bronchi as they approach the root. Central thoracic landmarks remain visible, including the left atrium receiving the pulmonary veins, the pulmonary trunk and pulmonary arteries, and the descending thoracic aorta along the left paravertebral line. That anatomy matters in thoracic surgery because venous drainage patterns in the left lower lobe often dictate safe planes for segmentectomy of basal segments and help prevent inadvertent sacrifice of adjacent intersegmental veins that can produce postoperative congestion or infarction. Preoperative CT angiography and intraoperative dissection around the posterior hilum rely on recognizing the relationship between the inferior pulmonary vein, the lower lobe bronchus, and the basal venous tributaries, a relationship that can vary and is a common source of orientation errors when working from a posterior approach. For pulmonary embolism workups, this view also helps separate venous pathways from the segmental pulmonary arteries that are interrogated on CT pulmonary angiography.
