Description

Positioned in posterior orientation, the right lung is shown with its bronchial tree and accompanying pulmonary vessels, with emphasis on the superior basal vein draining the superior segment (S6) of the lower lobe. The vessel courses from the posterior basal parenchyma toward the hilum, running in close relationship to the segmental bronchus and pulmonary artery branches before joining the inferior pulmonary vein. More centrally and inferiorly, the heart sits between the lungs, with the great vessels and pulmonary circulation rendered to keep the venous return pathway legible. Segmental pulmonary venous anatomy matters when you are trying to localize disease to a bronchopulmonary segment or plan a parenchyma-sparing resection. During right lower lobectomy or S6 segmentectomy, the superior basal vein is a common structure to identify and protect (or intentionally divide) to avoid venous congestion of preserved segments and to prevent inadvertent injury near the inferior pulmonary vein at the posterior hilum. This posterior view also aligns with what surgeons encounter when mobilizing the lower lobe and dissecting along the posterior mediastinum. Small structure, big consequences.
