Description
Running down the anterior aspect of the male upper limb, the median nerve descends from the brachial plexus into the arm, lying medial to the humerus and tracking closely with the brachial artery in the medial bicipital groove. Distal to the elbow, it passes into the anterior compartment of the forearm between the heads of pronator teres, then continues deep to flexor digitorum superficialis toward the wrist. The radius and ulna frame the neurovascular bundle proximally, while the carpals, metacarpals, and phalanges anchor the terminal distribution into the hand. Ulnar and radial nerve courses are included for orientation in the same anterior field. Clinically, this anterior pathway is where the median nerve is most often interrogated, first for entrapment at the pronator teres (pronator syndrome) and then for compression beneath the flexor retinaculum in carpal tunnel syndrome, producing paresthesia in the lateral three and a half digits and thenar weakness. Relationship to the brachial artery and the cubital fossa landmarks matters during venipuncture, arterial cannulation, and anterior approaches around the elbow, where iatrogenic nerve injury remains a real risk.
