Description

Centered in the anterior abdomen, the transverse colon spans from the right colic (hepatic) flexure to the left colic (splenic) flexure, with haustra, taeniae coli, and scattered appendices epiploicae modeled on its external surface. Within the transverse mesocolon, the middle colic vein courses superiorly and posteriorly from the mid transverse colon, accompanied by smaller venous tributaries draining the colonic wall. Proximally, this venous channel tracks toward the root of the mesentery to join the superior mesenteric vein en route to the portal venous system. Clear midgut venous anatomy. Middle colic venous drainage matters when you need to orient yourself around the superior mesenteric vessels and the inferior border of the pancreas, because the middle colic vessels mark the mesenteric window surgeons enter during a right hemicolectomy or transverse colectomy. Portal hypertensive colopathy, mesenteric venous thrombosis, and ischemic colitis can all involve this territory, and the pattern of middle colic tributaries helps explain why congestion or infarction may localize to the transverse colon. For trainees, it also reinforces the distinction between midgut venous return (via the superior mesenteric vein) and hindgut drainage (via the inferior mesenteric vein).
