Description
Cirrhotic change distorts the normal wedge-shaped liver into an irregular, nodular organ with a thickened Glisson capsule and coarse surface contour. Regenerative nodules protrude from the parenchyma, separated by pale fibrotic septa that would traverse from the subcapsular region toward the deeper lobular architecture, replacing the usual smooth convex diaphragmatic surface. Along the inferior aspect, the visceral surface landmarks (porta hepatis, gallbladder fossa, and fissures for the ligamentum teres and ligamentum venosum) become less crisp as bridging fibrosis contracts the tissue and rounds edges. Gross morphology matters because it reflects the underlying remodeling that drives portal hypertension and hepatic insufficiency. When fibrous bands partition lobules and create nodules, portal venous inflow meets increased resistance, setting up splenomegaly, ascites, and gastroesophageal varices, while distorted sinusoidal architecture contributes to jaundice, coagulopathy, and hepatic encephalopathy. Surface nodularity also carries a practical warning: in advanced cirrhosis, screening for hepatocellular carcinoma becomes mandatory, and focal lesions can be hard to distinguish from regenerative nodules without imaging correlation. Use this asset in hepatobiliary teaching for gross anatomy and pathology blocks, where learners need to connect lobular-level fibrosis with the palpable, bumpy liver encountered in autopsy, transplant explants, or intraoperative assessment during laparotomy and laparoscopy. It also fits clinical education on chronic hepatitis, alcohol-associated liver disease, and nonalcoholic steatohepatitis, and works well in review articles or patient-facing materials explaining why cirrhosis is not just scarring, but architectural collapse.
