Description
Radiating hepatic plates of hepatocytes form a hexagonal liver lobule centered on a prominent central vein (terminal hepatic venule). Blood-filled sinusoids course between the hepatocyte cords from the lobule periphery toward the center, converging into the central vein in a clear centripetal pattern. At each corner, portal areas contain the portal triad, a branch of the portal vein positioned alongside a smaller hepatic arteriole and an interlobular bile duct, with bile canaliculi running between adjacent hepatocytes in the opposite, centrifugal direction toward the duct. Orientation at this scale matters because it explains why zone 3 hepatocytes around the central vein are the first to suffer in ischemic injury and congestive hepatopathy, while zone 1 cells nearest the portal tracts face the highest exposure to blood-borne toxins. The same geometry underpins classic patterns such as centrilobular necrosis after acetaminophen toxicity and the pericentral fibrosis seen in chronic right-sided heart failure. Two flows, two outcomes. It also clarifies how mixed arterial and portal inflow enters the sinusoidal network and how obstruction at the sinusoid or central vein level contributes to portal hypertension and ascites.
