Description
Inferior hepatic anatomy comes into focus with the visceral (inferior) surface of a healthy liver, where the right and left lobes are separated anteriorly by the fissure for the ligamentum teres and posteriorly by the fissure for the ligamentum venosum. Medially, the quadrate lobe sits between the fossa for the gallbladder on the right and the fissure for the round ligament on the left, while the caudate lobe lies superior and posterior, bordered by the groove for the inferior vena cava. Posterior impressions for the stomach and esophagus tend to fall on the left lobe, with the renal and colic impressions more lateral on the right. Key landmarks. This perspective matters because surgeons and radiologists localize pathology on the visceral surface using these fixed fossae and fissures, then relate them to Couinaud segments for resection planning and reporting. The porta hepatis on the transverse fissure anchors the hepatoduodenal ligament, so the positional relationship of the hepatic artery proper, portal vein, and common bile duct in this region underpins the Pringle maneuver and helps explain patterns of biliary obstruction. When teaching normal hepatic parenchyma and lobule organization, a clean inferior surface also provides a practical external map for where intrahepatic vessels and bile ducts converge before entering the extrahepatic tract.
