Description

Oriented to the visceral (inferior) surface of a male liver, the parenchyma is presented with the classic H-shaped arrangement of fissures that subdivide the right and left anatomical lobes. Anteriorly the inferior border (margo inferior) sweeps across the field, while posteriorly the groove for the inferior vena cava lies deep and close to the bare area. Medially, the porta hepatis interrupts the visceral surface between the caudate lobe superiorly and the quadrate lobe inferiorly, and the fossa for the gallbladder occupies the right sagittal fissure just lateral to the quadrate lobe. Clear topography. For hepatobiliary teaching, the inferior view is the one that makes surgical anatomy feel concrete because it ties surface landmarks to the portal triad and venous outflow. Surgeons and trainees map the hepatoduodenal ligament to the porta hepatis during Pringle manoeuvre planning, and they relate the gallbladder fossa and cystic plate to safe dissection planes in laparoscopic cholecystectomy. Portal hypertension and hilar cholangiocarcinoma discussions also benefit from a clean depiction of the porta hepatis boundaries and the caudate lobe region, which can enlarge and distort the usual relationships.
