Seen from an inferior (visceral) perspective, the male liver presents the diaphragmatic organ’s undersurface with the quadrate lobe positioned anteroinferiorly, just medial to the gallbladder fossa and adjacent to the fissure for the ligamentum teres. Posteriorly, the caudate lobe sits superior to the porta hepatis, bounded by the groove for the inferior vena cava on the right and the fissure for the ligamentum venosum on the left, while the right and left hepatic lobes frame these central landmarks laterally. Impression surfaces for neighboring viscera are suggested along the visceral surface, including the gastric impression on the left lobe and the renal and colic impressions more laterally on the right. For teaching hepatic segmental anatomy, this underside view is where orientation often breaks down, because the porta hepatis, gallbladder fossa, and caval groove create the fixed landmarks that let you mentally map Couinaud segments and the course of the portal triad. It also matches the operative field in open and laparoscopic cholecystectomy, where the gallbladder bed on the visceral surface and the relationship of the quadrate lobe to the hilar plate matter when inflammation distorts planes. Small errors here cause big problems. Use this illustration in hepatobiliary surgery chapters to support discussion of the porta hepatis, hilar dissection, and caudate lobe access, and in gross anatomy or GI blocks when you need a clean reference for the visceral surface rather than the more familiar anterior diaphragmatic surface.
