Description
Centered in the right upper quadrant, the liver occupies the hypochondrium and extends medially across the epigastrium, with the right lobe projecting farther inferiorly than the left. Along the superior surface, the coronary ligament reflects from hepatic parenchyma to the undersurface of the diaphragm, and its lateral expansions form the right and left triangular ligaments at the lateral margins of the organ. The falciform ligament lies anteriorly in the midline, separating the right and left lobes superficially as it runs toward the anterior abdominal wall and diaphragm. Clear topography. An anterior presentation that calls out the coronary and triangular attachments matters when you are teaching liver mobility and the limits of safe retraction, because these peritoneal reflections define the bare area and tether the hepatic mass to the diaphragm. During open subcostal exposure or laparoscopic mobilization for right hepatectomy, trauma packing, or access to the retrohepatic inferior vena cava, dividing the right triangular ligament and opening the coronary ligament are routine steps, and students often confuse them with the falciform ligament seen from the front. This view keeps the ligamentous anatomy honest and prevents the common misconception that the liver is fixed uniformly to the anterior abdominal wall.
