Posterior surface anatomy places the liver deep to the right hemidiaphragm, occupying the right hypochondrium and extending medially into the epigastrium, with a small left lobe reaching toward the left upper quadrant. From the back, its superior (diaphragmatic) surface conforms to the domed diaphragm and sits immediately inferior to the right lung base, while the inferior margin slopes anteroinferiorly toward the costal margin. Relative to the vertebral column, the bulk of hepatic parenchyma lies right of midline, with the bare area positioned posteriosuperiorly where liver contacts diaphragm without intervening peritoneum. A quiet, clear landmark. This posterior, in situ presentation matters when you need to correlate surface landmarks with cross sectional imaging, because the liver’s dome rises high under the ribs and its apparent “upper border” shifts with diaphragmatic excursion between end expiration and deep inspiration. The relationship to the right pleura and lower lung explains how subphrenic collections or hepatic dome lesions can mimic thoracic pathology, and why intercostal approaches for percutaneous drainage or biopsy demand attention to pleural reflection and rib spaces. It also frames teaching around hepatic segments at the dome, where lesions in segments VII and VIII can be harder to access and are sometimes approached with a posterior oblique trajectory.
