Arising under the right hemidiaphragm, the male liver is rendered as an isolated organ with the convex diaphragmatic surface transitioning onto the visceral (inferior) surface, where the right lobe lies lateral to the falciform ligament and the smaller left lobe extends medially and superiorly toward the midline. On the underside, the quadrate lobe occupies an anteroinferior position between the fossa for the gallbladder on the right and the fissure for the ligamentum teres on the left, while the caudate lobe sits more posterior and superior, bordering the groove for the inferior vena cava. The porta hepatis separates these regions, marking the entry and exit point for the portal triad and hepatic ducts. Clear lobar boundaries make orientation straightforward. It is a clean teaching view. Attention to the quadrate and caudate lobes matters because these segments frame the hepatic hilum and major venous landmarks, so they are the first reference points when you correlate surface anatomy with Couinaud segmentation for hepatobiliary surgery. Caudate lobe hypertrophy is a familiar pattern in cirrhosis and Budd-Chiari syndrome, and its close relationship to the inferior vena cava explains why venous outflow obstruction and surgical control of short hepatic veins can be demanding. On imaging, the quadrate region is also where focal fatty sparing can mimic a mass near the gallbladder fossa. Misreading this area costs time. Use this rendering in gross anatomy and abdominal radiology teaching to anchor orientation before moving to CT and MRI, or in surgical atlases and patient education materials describing cholecystectomy landmarks and hepatic hilum anatomy.
