Description
Radiating hepatocyte plates form a hexagonal liver lobule centered on the central vein (terminal hepatic venule), with sinusoidal channels running from the lobular periphery toward that venous outflow. Portal tracts sit at the corners, where an interlobular branch of the portal vein lies alongside a hepatic arteriole and a bile ductule (cholangiolar component). Blood enters the sinusoids from portal venules and arterioles and courses centripetally to the central vein, while bile drains in the opposite direction through canaliculi toward the bile ductules. Countercurrent flow. Tracking the central vein as it approaches a larger hepatic vein branch matters because it ties microanatomy to the hemodynamics that drive classic zonal injury patterns. Zone 3 hepatocytes, adjacent to the central vein, see the lowest oxygen tension and are the first to undergo centrilobular necrosis in ischemic hepatitis and many drug toxicities (acetaminophen), while chronic passive congestion from right heart failure produces centrilobular sinusoidal dilatation and the familiar nutmeg pattern. The same spatial map explains why portal-based inflammation and fibrosis predominate in chronic viral hepatitis and many cholangiopathies, then bridge from portal tract to portal tract or portal tract to central vein as disease advances.
