Description
Running along the superomedial orbit, the angular vein forms at the medial canthus from the union of the supratrochlear and supraorbital veins and then descends inferiorly as the facial vein along the lateral aspect of the nose. Superiorly and posteriorly, venous channels track deep toward the orbit to communicate with the superior ophthalmic vein, which courses to the cavernous sinus through the superior orbital fissure. Superficial facial musculature such as orbicularis oculi, levator labii superioris alaeque nasi, and zygomaticus sits anterior to this venous pathway, while the nasal bones, maxilla, and medial orbital rim provide the bony framework deep to the soft tissues. Color coding separates venous structures from surrounding muscle and cartilage. Clinically, the angular vein matters because it links the superficial facial venous system with the orbit and intracranial venous sinuses via largely valveless channels. This is the anatomical basis for retrograde spread of infection from the medial canthus and upper lip, including septic cavernous sinus thrombosis after nasal vestibulitis or manipulation of a furuncle in the so called danger triangle of the face. Small vessel, big consequences. The same relationship also guides safe planes for periorbital injections and informs approaches to controlling facial bleeding in trauma.
