Description
Prominent within the colonic lumen, a pedunculated neoplastic polyp projects from the mucosal surface into the bowel cavity, its lobulated head positioned centrally and its stalk anchored to the colonic wall. The surrounding mucosa appears smoother and more uniform, forming the circular luminal contour that is partially encroached on by the mass. Fine, branching surface vessels course over the polyp, consistent with the vascularity of dysplastic mucosa and submucosa. Relative to the bowel axis, the lesion expands inward (toward the lumen) rather than outward toward the serosa, creating a focal site of luminal narrowing. Endoluminal morphology matters because colon cancer screening hinges on recognizing precursor lesions and early malignant change at colonoscopy, when mucosal contour, friability, and abnormal vascular patterns drive biopsy and resection decisions. A protruding adenoma can progress along the adenoma-carcinoma sequence, and as it enlarges it may cause occult bleeding, iron deficiency anemia, or intermittent obstruction, with the risk profile influenced by size, villous architecture, and high-grade dysplasia. Small changes in stalk thickness and head irregularity also affect technique selection for endoscopic mucosal resection and the assessment of possible submucosal invasion. Shape matters.
