Description
Posterior views of the male lower legs from the popliteal region to the plantar aspect emphasize the deep fascia (fascia cruris) as a dense connective tissue sheath investing the calf. Superficial to it, the gastrocnemius heads sit proximally and merge inferiorly toward the soleus, while the calcaneal (Achilles) tendon descends in the midline to insert on the posterior calcaneus. Laterally, the fibularis (peroneus) muscle mass tracks along the fibula, and intermuscular septa extend from the deep fascia toward the tibia and fibula, partitioning posterior and lateral compartments. Fascia cruris matters because its unyielding septa and the transverse intermuscular septum create closed osteofascial spaces that drive the pathophysiology of acute compartment syndrome after tibial shaft fracture, crush injury, or reperfusion. Pressure rises, capillary perfusion falls. This posterior emphasis also supports teaching the surgical logic of fasciotomy, including release of the superficial and deep posterior compartments and the relationship of the deep posterior space to the neurovascular bundle (posterior tibial artery and tibial nerve) lying deep to soleus.
