Description
Posterior lower legs are rendered with a split-layer approach: one side emphasizes the superficial posterior compartment with the gastrocnemius (medial and lateral heads) overlying the soleus, while the contralateral limb is dissected to the plane of the deep transverse fascia (transverse intermuscular septum) separating superficial from deep calf musculature. Inferiorly, the triceps surae converge into the calcaneal (Achilles) tendon, which courses distally to insert on the posterior calcaneal tuberosity. Deep to the fascial plane, the deep posterior compartment structures are logically exposed, including flexor hallucis longus laterally, flexor digitorum longus medially, and tibialis posterior in the intermediate plane, with their tendons tracking toward the tarsal tunnel posterior to the medial malleolus. Clear compartmental anatomy. That transverse fascia matters when you need to explain why calf pathology does not behave uniformly across the shank (crus). In acute compartment syndrome of the leg, pressure rises within a specific compartment bounded by dense fascia, and the deep posterior compartment is a frequent diagnostic blind spot on exam, even while the superficial gastrocnemius-soleus complex looks relatively unaffected. The same fascial relationships guide posterior approaches and fasciotomy planning, and they frame the course of posterior tibial neurovascular structures that can be threatened by swelling, deep hematoma, or iatrogenic dissection.
