Description
Within a skull cross section of an adult male, the episcleral vein is traced in the superficial vascular plexus overlying the sclera, just deep to the bulbar conjunctiva and continuous anteriorly with the limbal region. Small episcleral venules converge into larger collecting channels that run circumferentially around the globe, lying anterior to the equator and superficial to the insertions of the rectus muscles. Posteriorly, the optic nerve exits the globe at the optic disc and courses through the orbital apex toward the cranial cavity, framed by the bony orbit and adjacent cranial base structures. Episcleral venous anatomy matters because it is the final low pressure outflow bed for aqueous humor after passage through Schlemm’s canal and the collector channels, so changes in episcleral venous pressure can translate into elevated intraocular pressure.
