Description
Seen in section through the craniofacial skeleton, the orbit is opened to the globe and anterior optic pathway, with the episcleral artery coursing as a tortuous red vessel on the superficial sclera deep to the bulbar conjunctiva. Anteriorly it arcs around the limbus at the corneoscleral junction, while posteriorly it relates to the rectus muscle insertions and the episcleral venous plexus on the globe’s surface. Medial orbital walls and the nasal cavity sit medial to the orbit, and the optic nerve runs posteriorly from the globe toward the optic canal and cranial cavity. Bony landmarks of the maxilla and upper dentition remain inferior and anterior. Episcleral vessels matter because they are the visible, accessible link between the anterior ciliary circulation and the conjunctival and intrascleral vascular beds that influence intraocular pressure. Sectoral episcleral injection helps differentiate episcleritis from scleritis, and that clinical distinction drives workup for systemic inflammatory disease and urgency of treatment. In glaucoma, elevated episcleral venous pressure can limit aqueous outflow through Schlemm’s canal, while conjunctival and episcleral handling during filtration surgery or tube shunt placement can determine bleb function.
