Description
Centered within a cranial cross section, the globe sits in the bony orbit with the sclera forming the outer coat and the optic nerve exiting posteriorly at the optic disc toward the orbital apex. Anterior ciliary arteries are shown coursing along the anterior surface of the globe near the limbus, running within the episcleral tissues and approaching the rectus muscle insertions before contributing to the anterior segment circulation. Arterioles branch circumferentially across the anterior sclera, while companion venous channels drain posteriorly within the episcleral and conjunctival plexuses. Portions of the orbital walls and adjacent paranasal sinus cavities frame the eye medially and inferiorly. Emphasis on the anterior ciliary arteries matters because these vessels, arising from the muscular branches of the ophthalmic artery, form a major supply to the anterior uvea through the episcleral arterial circle and contributions to the major arterial circle of the iris. This is the vascular territory challenged in strabismus surgery: disinsertion of multiple rectus muscles can compromise anterior segment perfusion and, in susceptible patients, precipitate anterior segment ischemia with corneal edema, anterior chamber reaction, and a poorly reactive pupil. Small arteries, big consequences. Ophthalmology and head and neck anatomy courses can use this section to teach anterior segment blood supply alongside orbital landmarks and the relationship of the orbit to the ethmoid and maxillary sinuses.
