Description
Presented in a sectional skull context, the globe sits within the orbit with the sclera forming a thick outer coat and the optic nerve exiting posteriorly at the optic disc. Multiple short posterior ciliary arteries branch from the ophthalmic artery via the posterior ciliary circulation and approach the posterior pole, then pierce the sclera in a tight ring around the optic nerve, immediately proximal to the optic axis. Smaller accompanying veins and periorbital vessels track in parallel, while the bony orbit frames the eye laterally and superiorly and the adjacent nasal cavity lies anteromedial to the orbital contents. Short posterior ciliary arteries are the dominant arterial supply to the choroid and prelaminar portion of the optic nerve head, so their pattern and point of scleral penetration matter when you are teaching or thinking about posterior segment ischemia. Compromise of this circulation underlies the optic disc edema and sudden vision loss typical of non-arteritic anterior ischemic optic neuropathy, and it also provides the anatomical substrate for the segmental choroidal hypoperfusion seen in giant cell arteritis.
