Description
Seen from a superior aspect through a skull cross section, the orbits are opened to reveal the globes with the optic nerves (optic stalks) coursing posteriorly toward the optic canals. Surrounding the nerve at its posterior scleral entry, a tight cluster of short posterior ciliary arteries fans forward from the ophthalmic artery territory, traveling in a circumferential pattern around the optic nerve and then penetrating the sclera to supply the choroid and optic nerve head. Superiorly placed extraocular structures and neurovascular elements track along the orbital apex, with arterial and venous channels color-coded red and blue to clarify relationships at the orbital roof and intracranial margin. Small vessels dominate the field. This superior perspective clarifies why the short posterior ciliary arteries matter to both ocular perfusion and neuro-ophthalmic disease: they form the principal input to the peripapillary choroid and contribute to the circle of Zinn Haller around the optic nerve head, a vascular arrangement implicated in anterior ischemic optic neuropathy. Their close proximity to the optic nerve at the orbital apex also explains vulnerability during surgery or trauma near the optic canal, and why even minor compression can compromise choroidal blood flow. Surgical anatomy reads differently from above, and this angle makes the arterial cluster easier to track. Use this illustration in head and neck anatomy courses when teaching the orbital apex, in ophthalmology chapters on the vascular supply of the choroid and optic disc, or in neuro-ophthalmology content discussing optic nerve head ischemia and perioperative risk around the optic canal.
