Description
Oblique anatomy of the ankle and foot is rendered with the distal tibia and fibula superiorly framing the talus, which sits atop the calcaneus to form the tibiotalar and subtalar joints. Distal to the hindfoot, the navicular lies medial to the cuboid, and the tarsometatarsal (Lisfranc) articulations align the cuneiforms and cuboid with the bases of the metatarsals as they continue distally to the phalanges. Posteriorly, the calcaneal (Achilles) tendon inserts on the calcaneal tuberosity, while anterior and plantar tendons course toward the midfoot and forefoot in layered planes. Neurovascular structures track with expected topography, including the posterior tibial artery and tibial nerve posteromedial to the medial malleolus and the dorsalis pedis artery on the dorsum of the midfoot. This perspective is the one you want when teaching how bony architecture and soft tissue restraints cooperate at the transverse tarsal (talonavicular and calcaneocuboid) complex and at Lisfranc’s joint. It also maps cleanly onto common clinical problems: posterior tibial tendon dysfunction with acquired flatfoot, Achilles tendinopathy at the calcaneal insertion, and entrapment within the tarsal tunnel where the tibial nerve and posterior tibial vessels pass beneath the flexor retinaculum. Fracture-dislocations of the midfoot become easier to explain when the relationship between the cuneiforms, cuboid, and metatarsal bases is explicit. A hard-working region.
