Description
Fan-shaped genioglossus fibers arise from the superior mental spines (genial tubercles) on the lingual surface of the anterior mandible and sweep posteriorly and superiorly into the body of the tongue. In lateral section, the muscle belly lies deep to the mandibular symphysis and oral mucosa, forming much of the tongue base anterior to the oropharyngeal airway, with the hyoid bone and laryngeal framework positioned inferior and posterior. The cervical vertebrae align posterior to the pharynx, and the maxilla, dentition, and temporomandibular region provide bony context anteriorly and superiorly. Orientation is clear. Genioglossus is the primary tongue protrusor and a key stabilizer of the anterior airway, so a lateral head and neck perspective helps you track how its anterior mandibular attachment translates into posterior tongue-base bulk that can narrow the hypopharynx when tone is reduced. Sleep-related airway obstruction often localizes to the retrolingual space, and this is the muscle targeted indirectly by mandibular advancement devices and directly by hypoglossal nerve stimulation; paresis of cranial nerve XII can also produce tongue deviation and impaired bolus propulsion. Surgical planning for transoral approaches benefits from seeing the relationship between the mandibular symphysis, tongue musculature, hyoid level, and the laryngeal inlet.
