Description
Arising beneath the right hemidiaphragm, the male liver occupies the right upper quadrant and extends medially across the epigastrium, framed superiorly by the costal cartilages and posteriorly by the thoracic vertebral column. The inferior (visceral) surface is oriented toward the upper abdominal viscera, where the quadrate lobe lies anteroinferior and medial, bounded by the gallbladder fossa laterally and the fissure for the ligamentum teres medially. Posterior and superior to this, the caudate lobe sits adjacent to the groove for the inferior vena cava, separated from the left lobe by the fissure for the ligamentum venosum, while the porta hepatis crosses transversely between caudate and quadrate regions. For teaching surface and surgical anatomy, this configuration matters because the porta hepatis is the gateway to the portal triad (hepatic portal vein posteriorly, proper hepatic artery anteromedially, and bile duct anterolaterally), and its relationship to the caudate and quadrate lobes guides safe dissection during hepatobiliary surgery. The caudate lobe warrants special attention in cirrhosis and Budd-Chiari syndrome, where caudate hypertrophy can occur because its venous drainage into the inferior vena cava is partly independent of the major hepatic veins. Small structures, big consequences.
