Description
Dominating the lateral neck is the middle pharyngeal constrictor, a fan-shaped sheet arising from the greater and lesser horns of the hyoid bone and sweeping posteriorly to the median pharyngeal raphe. Superior to its upper border lie the inferior fibers of the superior pharyngeal constrictor, while its inferior margin overlaps the upper part of the inferior pharyngeal constrictor at the level of the thyrohyoid membrane. Anteriorly, the muscle blends with the pharyngeal wall deep to the tongue base and suprahyoid region, with the mandible and lower skull providing bony orientation, and cervical vertebrae forming the posterior skeletal axis. Functionally, the middle pharyngeal constrictor narrows the pharyngeal lumen during the pharyngeal phase of swallowing, moving the bolus inferiorly toward the esophageal inlet in coordination with palatal elevation and laryngeal excursion. This lateral perspective helps clarify the clinically relevant overlap zones between constrictors and the relationship to the hyoid complex, where postoperative scarring after hyoid suspension, neck dissection, or transoral procedures can contribute to dysphagia.
