Description
Spanning the anterolateral neck as a thin subcutaneous sheet, the platysma lies superficial to the sternocleidomastoid and the strap muscles, blending superiorly with the superficial musculoaponeurotic system along the inferior border of the mandible. Medially, right and left platysma fibers approach the midline over the laryngeal prominence and suprahyoid region, while laterally the muscle expands toward the parotid-masseteric fascia and the skin of the cheek. Inferiorly, its fibers arise from the superficial fascia over the upper thorax and shoulder, then course superomedially toward the lower face. A broad, superficial depressor. For head and neck teaching, this muscle is less about force generation and more about surgical planes and aging anatomy: the platysma defines the superficial cervical fascia, and its anterior decussation (or lack of it) influences midline banding seen with facial aging. Surgeons track it during rhytidectomy and neck lift procedures, where platysmaplasty targets medial platysmal bands and the subplatysmal space provides access to the submandibular gland, anterior jugular veins, and cervical branches of the facial nerve. Trauma and oncologic neck dissections also reference the platysma as an early landmark, since incision depth relative to this layer changes both exposure and risk to superficial veins and cutaneous nerves. Use this illustration in gross anatomy and surface anatomy courses when introducing superficial fascia of the neck, facial expression musculature, and surgical layers from skin to investing cervical fascia. It also fits well in plastic surgery and otolaryngology publications discussing cervicofacial rejuvenation, submandibular approaches, and the anatomy underlying platysmal banding in men.
