Description

Sectioned through the kidney in an anterior cross section, the renal cortex forms a peripheral rim around the medulla, where renal pyramids converge toward the papillae and empty into the minor calyces and renal pelvis. Fine renal lymphatics course within the fibrous stroma, tracking along interlobar and arcuate vessels at the corticomedullary junction and continuing toward the renal sinus. Along the hilum, these lymphatic vessels accompany the renal artery and renal vein, then exit toward paraaortic (lumbar) lymph nodes. Small channels also lace the renal capsule and perirenal connective tissue. Delicate vessels. Renal lymphatic anatomy matters when you need to explain how disease leaves the kidney. Lymph from the capsule, cortex, and renal sinus provides a pathway for renal cell carcinoma spread to paraaortic nodes, and it also helps account for retroperitoneal nodal enlargement patterns seen on staging CT. This sectional plane supports teaching of lymphatic drainage in relation to the collecting system and hilar vessels, which is also relevant during nephrectomy and hilar dissection where lymphatic disruption can contribute to postoperative lymphocele. Use this illustration in gross anatomy and urogenital blocks to pair renal parenchymal organization (cortex, medulla, pyramids, calyces, pelvis) with the less commonly diagrammed renal lymphatics and their relationship to the hilum.
