Splenic Vein

Description

Arising from the splenic hilum in the left upper quadrant, the splenic vein courses medially and slightly superior to the left kidney, then tracks toward the midline as it runs immediately posterior to the body and tail of the pancreas. An anterior perspective permits simultaneous orientation to the stomach and spleen anterolaterally, with the abdominal aorta and inferior vena cava positioned centrally and posteriorly against the vertebral column. Near the pancreatic neck, the splenic vein approaches the superior mesenteric vein to form the portal vein, with the liver visible superiorly on the right under the rib cage. Relations are clear. For clinicians, the key teaching point is the vein’s intimate retropancreatic course, which explains why pancreatic inflammation and pancreatic carcinoma frequently involve splenic vein thrombosis. Segmental (left-sided) portal hypertension can follow, producing isolated gastric varices while the main portal vein and liver parenchyma may remain otherwise unremarkable. This anterior anatomic layout also supports operative planning for distal pancreatectomy and splenectomy, where control of the splenic vessels near the pancreatic tail reduces hemorrhage risk but puts the pancreas at risk of iatrogenic injury. Ideal use cases include gross anatomy and GI/hepatobiliary modules that cover the portal venous system, as well as surgical atlases and review articles on pancreatitis-related venous complications, gastric varices, and splenic vein thrombosis. It also reads well in radiology teaching files when correlating CT or MR venography findings with expected vessel position behind the pancreas.
