Description
Running immediately inferior to the twelfth rib, the subcostal nerve (anterior ramus of T12) tracks laterally from the thoracolumbar junction across the posterior abdominal wall before continuing into the anterolateral abdominal wall. From an anterior perspective, the lower thoracic cage with intercostal spaces frames the nerve’s proximal course, while the abdominal wall musculature, including rectus abdominis medially and the layered anterolateral muscles laterally, defines its distal pathway. Branches are shown coursing between muscle planes, consistent with its relationship to the transversus abdominis and internal oblique. Landmarks matter. Because T12 bridges thoracic and abdominal wall innervation, this view helps clarify why “subcostal” pain or sensory change can be mistaken for lower intercostal neuralgia or upper lumbar radiculopathy. The subcostal nerve is frequently encountered during flank and retroperitoneal exposure, including open or laparoscopic approaches near the 12th rib, where traction, trocar placement, or rib resection can produce abdominal wall paresis or a localized bulge from denervation of the lateral abdominal muscles.
