Emerging segmentally from the thoracic spinal nerves, the intercostal nerves course laterally from the intervertebral foramina beneath each rib, running in the costal groove inferior to the corresponding rib and deep to the intercostal musculature. From an anterior oblique viewpoint, the nerves can be followed from their proximal roots near the thoracic vertebrae toward the anterolateral thoracic wall, where each gives off a lateral cutaneous branch near the midaxillary line and then continues anteriorly to terminate as an anterior cutaneous branch beside the sternum. Posteriorly placed landmarks such as the scapulae sit superficial to the rib cage, while the thoracic and upper lumbar vertebrae and intervertebral discs anchor the segmental pattern. Clinically, this layout mirrors the pain distribution in thoracic radiculopathy and herpes zoster, where symptoms track a single intercostal nerve from paravertebral origin to the anterior chest wall. The branching pattern also matters for procedures: intercostal nerve blocks target the neurovascular plane between the internal and innermost intercostal muscles near the inferior rib margin, and surgical portals for VATS or chest tube placement aim to avoid the main neurovascular bundle while accounting for variable lateral cutaneous branches.
