Description
Arcing along the lateral thoracic wall, the intercostal nerves emerge segmentally from the thoracic spinal nerves just distal to the dorsal root ganglia, then run anteriorly and inferiorly within each intercostal space beneath the corresponding rib. From this lateral perspective, the nerves can be read in relationship to the thoracic vertebrae posteriorly, the rib heads and costotransverse region, and the curving shafts of the ribs as they sweep toward the anterior chest. The scapula sits posterolaterally over the upper ribs, providing an external landmark that helps orient the posterior axillary line relative to the intercostal spaces. Clinically, this is the course that matters for intercostal nerve blocks and chest tube placement, because the neurovascular bundle travels in the costal groove along the inferior margin of the rib, with the nerve typically inferior to the intercostal artery and vein.
