Description
Running along the inferior margins of each rib, the intercostal nerves trace a segmental course through the intercostal spaces from the thoracic spinal nerves, accompanied by adjacent intercostal vessels. Posteriorly, the nerves emerge near the thoracic vertebrae and sympathetic chain ganglia, then pass laterally between the ribs toward the costal cartilages and sternum. A slightly lateral view of the male thoracic cage keeps the rib arcs, costovertebral joints, and the parasternal region in the same field, with one scapula silhouetted on the posterolateral chest wall. The pattern is repetitive, but not uniform. Clinically, this is the map you want for local anesthesia and pain syndromes of the chest wall. Intercostal nerve blocks and thoracostomy tube placement both depend on remembering that the neurovascular bundle runs in the costal groove, so instruments and needles are directed just superior to the rib below to reduce risk of nerve injury and bleeding. The segmental origin also explains band-like radicular pain from thoracic disc disease and the dermatomal distribution of herpes zoster, while the proximity to the sympathetic trunk frames why thoracic sympathetic blocks can alter sweating and vasomotor tone.
