Description
Inferiorly oriented anatomy of the male floor of mouth and upper neck centers on the paired submandibular glands tucked against the medial surface of the mandibular body, with the mylohyoid muscle forming a muscular sling superior to them and the anterior belly of the digastric running anteroinferiorly toward the digastric fossa. Medial to the gland, the hyoglossus and genioglossus define the submandibular space and the deep lobe’s relationship to the tongue base, while Wharton’s (submandibular) duct courses anteriorly toward the sublingual caruncle. Expect the facial artery to groove the posterior aspect of the gland as it loops over the mandible, with the submandibular vein and regional lymph nodes lying more superficial and inferior in the submandibular triangle. Spatial relationships matter here. For teaching and operative planning, this underside perspective clarifies why submandibular gland excision demands respect for the marginal mandibular branch of the facial nerve near the inferior border of the mandible and the lingual and hypoglossal nerves deep to the gland and duct. Sialolithiasis most often obstructs Wharton’s duct, and knowing its anterior course along the floor of mouth helps explain the classic bimanual palpation findings and why ductal stones cluster near the hilum. The submandibular lymph nodes shown in proximity also frame patterns of metastatic spread from oral cavity and anterior tongue primaries.
