Description
Running longitudinally along the posterior aspect of the spinal cord, paired posterior spinal arteries course inferiorly and give off circumferential branches that wrap laterally toward the anterior surface to anastomose with the anterior spinal artery in the midline. Radiculomedullary (segmental) arteries enter with the spinal nerve roots through the intervertebral foramina, then join the pial arterial plexus (vasocorona) within the subarachnoid space, immediately superficial to the pia mater. A denser, more tortuous blue venous network parallels the arteries and drains via radiculomedullary veins toward the internal vertebral venous plexus within the vertebral canal. Vertebral elements of the lower spine form the bony backdrop, helping orient the vessels relative to the vertebral canal and dorsal roots. Posterior emphasis matters because the dorsal columns and dorsal horns tolerate ischemia poorly, and their blood supply depends on small-caliber pial branches that are easily compromised by hypotension, vasculitis, or iatrogenic injury. This is the anatomy behind classic anterior spinal artery syndrome versus more selective posterior cord infarction, and it also frames why radiculomedullary reinforcement, including the artery of Adamkiewicz (usually left-sided, lower thoracic to upper lumbar), is so carefully sought on spinal angiography before thoracoabdominal aortic repair. Even in a stylized rendering, the relationship of vessels to the nerve roots is a practical landmark for surgeons and interventionalists.
