Description
Posterior female musculature is rendered from the occiput to the calcanei, with the trapezius spanning from the nuchal region and thoracic spinous processes to the lateral clavicle and acromion, and the deltoid capping the glenohumeral joint laterally. Medial to the scapular borders, the rhomboid major and minor sit deep to trapezius, while the latissimus dorsi sweeps inferolaterally from the thoracolumbar fascia and iliac crest toward the proximal humerus, framing the posterior axillary fold. Along the vertebral column the erector spinae (iliocostalis, longissimus, spinalis) forms paired longitudinal columns, continuing inferiorly into the gluteal region where gluteus maximus overlies the posterior hip and the hamstrings (biceps femoris, semitendinosus, semimembranosus) descend to the knee, with gastrocnemius and soleus forming the calf and converging into the calcaneal (Achilles) tendon distally. Clean fascial planes are visible. Posterior muscle anatomy is the map for common pain patterns and operative corridors. Palpation and trigger point referral often track from the upper trapezius and levator scapulae into the cervicothoracic junction, while scapular dyskinesis frequently reflects imbalance between trapezius and the scapular retractors that stabilize the medial border during shoulder elevation. In the lumbar region, surgeons splitting the thoracolumbar fascia and dissecting through the paraspinal musculature during posterior decompression or fusion must respect the segmental innervation and vascular supply to limit postoperative denervation atrophy. Injection and flap planning rely on it.
