Description
Arising from the olfactory bulbs on the inferior surface of the frontal lobes, the olfactory nerve (cranial nerve I) is represented as multiple fila olfactoria that descend through the foramina of the cribriform plate of the ethmoid to reach the olfactory mucosa in the superior nasal cavity. Medially, the olfactory sulcus and the interhemispheric fissure provide midline orientation, while laterally the orbital gyri frame the bulbs and tracts along the basal frontal surface. Short olfactory tracts course posteriorly from each bulb toward the olfactory trigone, sitting anterior to the region where the anterior cerebral arteries run on the basal forebrain. Fine anatomy. Clinical relevance is straightforward: shearing of these delicate filaments at the cribriform plate is a classic mechanism for post traumatic anosmia after anterior skull base injury, often accompanied by cerebrospinal fluid rhinorrhea when the adjacent meningeal barrier is disrupted. Endoscopic sinus surgery and transcribriform approaches also demand precise awareness of this inferior relationship between the olfactory apparatus and the ethmoid roof, because even minor violation of the fovea ethmoidalis or cribriform region can permanently affect smell. This inferior perspective makes the anatomic choke point obvious.
