Description
Running in the substance of the soft palate, the palatine aponeurosis forms a fibrous sheet anchored anteriorly to the posterior margin of the hard palate (palatine processes of the maxilla and horizontal plates of the palatine bones) and draped posteriorly toward the free border of the velum. In this sagittal male head and neck section, it lies inferior to the nasal cavity and superior to the oral cavity, separating nasopharynx from oropharynx at rest. The aponeurosis blends laterally with the tendinous expansion of tensor veli palatini and provides a firm insertion for levator veli palatini, while the uvula projects from the posterior midline of the soft palate. That fibrous plate matters because it is the mechanical backbone of velopharyngeal closure, the action that seals the nasopharynx during swallowing and most speech sounds. Disruption or laxity of the aponeurosis, as in submucous cleft palate or after palatal surgery, leads to velopharyngeal insufficiency with hypernasal speech and nasal regurgitation, and this sagittal relationship makes the failure mode easy to understand. It is also a key landmark when teaching why the tensor veli palatini tendon hooks around the pterygoid hamulus to tense the palate.
