Description
Seen from a posterior perspective, the right and left lobes of the thyroid gland frame the upper trachea and lower larynx, with the posterior thyroid capsule and adjacent pretracheal fascia forming the surface on which the parathyroids sit. Superior parathyroid glands typically lie near the posterior aspect of the upper thyroid lobes, close to the cricothyroid junction, while the inferior parathyroid glands sit more caudally and variably, often near the inferior poles. Small ovoid nodules contrast against the thyroid parenchyma. Spatial orientation is clear: the parathyroids remain posterior to the thyroid lobes and lateral to the tracheal midline. Posterior anatomy matters most in thyroid and parathyroid surgery, because the parathyroid glands and their end-arterial supply run in tight quarters with the recurrent laryngeal nerve in the tracheoesophageal groove. Injury or devascularization during thyroidectomy leads to postoperative hypocalcemia, and this view helps you anticipate where a “missing” inferior parathyroid may have migrated along the thyrothymic tract. It also supports teaching the embryology-driven difference in typical position and variability between superior and inferior glands.
