Presented from a plantar perspective beneath the skin, the oblique head of the adductor hallucis spans the midfoot from the bases of the second to fourth metatarsals and adjacent tarsal ligaments toward its distal insertion on the lateral aspect of the proximal phalanx of the hallux, blending with the lateral sesamoid apparatus. Medially, the first metatarsal and hallux phalanges frame the muscle’s course, while the lesser metatarsals lie more lateral and proximal to its origin. The calcaneus and talus anchor the hindfoot posteriorly, and blue cartilage markings delineate the metatarsophalangeal and interphalangeal joint surfaces. Fiber direction matters. Functionally, this muscle is the main soft tissue counterforce to the abductor hallucis across the first metatarsophalangeal joint, so its line of pull becomes clinically conspicuous in hallux valgus and bunion deformity where lateral drift of the hallux is reinforced. Surgeons addressing deformity often release or balance the adductor hallucis tendon and lateral sesamoid complex, and this plantar view clarifies what sits immediately superficial and deep to the lateral capsule and sesamoids. In the setting of metatarsalgia, appreciating the muscle’s relationship to the transverse metatarsal region helps explain why overload and altered forefoot mechanics can produce pain even when radiographs look unremarkable.
