Description
Beneath the skin of the male nose, the posterior dilator naris is rendered as a narrow slip of muscle within the alar (wing) soft tissues, positioned lateral to the external naris and superficial to the alar cartilage. Its fibers blend with the nasalis complex at the level of the ala nasi, lying anterior to the maxillary bone and inferior to the lower margin of the nasal bone. Surrounding facial musculature is also exposed, including orbicularis oculi encircling the palpebral fissure, zygomaticus major and minor coursing inferomedially from the zygomatic bone to the oral commissure, and orbicularis oris forming the sphincter of the mouth. Temporalis occupies the temporal fossa superiorly, while sternocleidomastoid descends obliquely along the anterolateral neck. Orientation matters here because small perinasal muscles are frequent sources of confusion in surface anatomy and in operative dissection, where their fibers interdigitate and the terms “dilator naris,” “alar part of nasalis,” and “levator labii superioris alaeque nasi” get misapplied. The posterior dilator naris contributes to alar flaring and maintenance of the nasal valve region, a point of interest in functional rhinoplasty and in reconstruction after alar rim trauma or cleft-related deformity, where subtle changes in muscular balance can worsen inspiratory collapse. Red emphasis over the ala highlights the anatomic neighborhood where surgeons often raise thin skin-soft tissue flaps and risk denervating or scarring these fine fibers. Plastic surgery and ENT training programs can pair this plate with dissection lab material to teach the alar musculature during rhinoplasty planning, as well as in modules on facial expression muscles and their innervation by the buccal and zygomatic branches of the facial nerve.
