Description
Posterior thoracic wall anatomy is presented with the rib cage reflected to expose the posterior intercostal veins coursing in each intercostal space and converging medially toward the vertebral column. Segmental veins run inferior to their corresponding ribs, paralleling the posterior intercostal arteries and the intercostal nerves within the costal groove, then pass toward the heads and necks of the ribs adjacent to the thoracic vertebrae. Near the vertebral bodies, the vessels align with the azygos system, with right posterior intercostal veins draining into the azygos vein and left-sided channels typically joining the hemiazygos and accessory hemiazygos veins before crossing to the right. Clean midline landmarks. Understanding this posterior perspective matters when you need to teach or plan around vertebral-level venous drainage and its common variants. Posterior intercostal veins communicate with the internal vertebral venous plexus, a route implicated in hematogenous spread of infection or metastasis to the spine, and they are encountered during thoracotomy, rib resection, and posterior approaches near the costovertebral junction where bleeding can be brisk. The segmental pattern also anchors interpretation of posterior mediastinal anatomy on CT and in cross-sectional teaching, where the azygos arch and hemiazygos connections provide orientation. Use this illustration in gross anatomy labs for thorax and posterior body wall, in radiology teaching files that correlate vertebral levels with azygos and hemiazygos anatomy, and in surgical education materials covering posterolateral thoracotomy, chest wall trauma, and intercostal vessel control. It also fits well in publisher plates on venous collateral pathways in superior vena cava obstruction and posterior mediastinal pathways.
