Arising from the posterior tubercle of the atlas (C1), the rectus capitis posterior minor runs superiorly and slightly medially to insert on the medial portion of the inferior nuchal line of the occipital bone, immediately inferior to the external occipital protuberance. From a lateral perspective, its short belly sits deep in the suboccipital region, posterior to the atlanto-occipital joint and just medial to the larger rectus capitis posterior major. The occipital squama, the posterior arch of C1, and the upper cervical curvature frame the muscle’s course. Small. Delicate. This specific muscle matters because it anchors the posterior atlanto-occipital complex and blends with the posterior atlanto-occipital membrane, a relationship often discussed in the context of the myodural bridge and cervicogenic headache mechanisms.
