Description
Positioned on the posterior aspect of the subtalar region, the posterior talocalcaneal ligament spans from the posterolateral talus (near the posterior process) to the superior posterior calcaneus, crossing the back of the talocalcaneal joint in short, obliquely oriented fibers. The distal tibia and fibula form the ankle mortise superiorly, with the talus seated between the malleoli and the calcaneus lying inferior and slightly posterior as the heel. A narrow fat-filled interval between talus and calcaneus corresponds to the posterior part of the sinus tarsi and tarsal canal region, adjacent to the posterior subtalar joint line. Posterior subtalar restraints are easy to hand-wave in teaching, but they matter when you are explaining hindfoot stability and subtalar arthrokinematics. The posterior talocalcaneal ligament limits excessive posterior translation and rotational play between talus and calcaneus during inversion and eversion, and it sits close to structures targeted in hindfoot arthroscopy and subtalar arthrodesis, where posterior portals and screw trajectories demand a clean mental map of what lies between the talar process and calcaneal tuberosity.
