Positioned on the posterior aspect of the subtalar region, the posterior talocalcaneal ligament spans from the posterolateral talus (near the posterior process) to the superior posterior calcaneus, crossing the back of the talocalcaneal joint in short, obliquely oriented fibers. The distal tibia and fibula form the ankle mortise superiorly, with the talus seated between the malleoli and the calcaneus lying inferior and slightly posterior as the heel. A narrow fat-filled interval between talus and calcaneus corresponds to the posterior part of the sinus tarsi and tarsal canal region, adjacent to the posterior subtalar joint line.
