Description
Posteriorly, the distal tibia lies medial and the distal fibula lies lateral, framing the talus at the talocrural (ankle) mortise. The calcaneofibular ligament descends from the apex of the lateral malleolus in an inferoposterior direction to insert on the lateral surface of the calcaneus, positioned posterior to the subtalar joint line and inferior to the fibular facet of the talus. Articular margins of the tibiotalar and subtalar joints are indicated by cartilage or joint space markings, clarifying the relationship between the ligament and the hindfoot articulations. Lateral ankle stability is often taught as a triad of the anterior talofibular, calcaneofibular, and posterior talofibular ligaments, but the calcaneofibular component is the one that spans both the tibiotalar and subtalar joints. That anatomy explains why calcaneofibular injury in an inversion sprain correlates with subtalar instability and persistent hindfoot pain, and why stress testing in dorsiflexion can better tension this band than testing in plantarflexion.
