Description
Running in the midline from the lower thoracic spinous processes toward the sacrum, the supraspinous ligament forms a continuous fibrous band superficial to the interspinous ligaments and deep to the skin and subcutaneous fascia. From this posterior perspective, it sits between the right and left erector spinae muscle masses, with multifidus lying deeper and more medial, and it blends laterally with the posterior layer of the thoracolumbar fascia over the lumbar region. Inferiorly, the ligamentous midline relates to the dorsal surface of the sacrum and the posterior iliac crests, where the thoracolumbar fascial aponeuroses converge. A clear midline landmark. That close fusion between supraspinous ligament and thoracolumbar fascia matters when you teach or plan posterior lumbar approaches, because the midline fascial raphe and intermuscular cleavage plane guide subperiosteal dissection off the spinous processes and laminae. Degenerative change or strain in this posterior tension band contributes to midline low back pain and can coexist with interspinous bursitis (Baastrup disease), where adjacent spinous processes approximate and irritate the interspinous region.
