Description
Posterior midline anatomy dominates the scene, with the supraspinous ligament rendered as a dense, cord-like band running longitudinally along the apices of the spinous processes from the lower cervical region through the thoracic and lumbar spine toward the sacrum. Deep to it, the interspinous ligaments occupy the interspinous spaces, and the ligamentum flavum bridges adjacent laminae more anteriorly, defining the posterior boundary of the vertebral canal. Laterally, the erector spinae mass flanks the spinous processes, while the supraspinous fibers blend superiorly with the nuchal ligament and inferiorly approach the posterior sacral crest. Short and tough. That longitudinal continuity matters when you are teaching the posterior tension band and why flexion stresses the posterior ligaments while extension approximates spinous processes. Hyperflexion injuries can produce interspinous widening and supraspinous rupture, a pattern often flagged on MRI in thoracolumbar trauma and used to judge stability in AO spine classifications. Palpation and landmarking also start here, since the ligament tracks the tips of palpable spinous processes and helps orient midline approaches.
