Description
Running transversely across the distal anterior forearm, the pronator quadratus is shown from a medial perspective as a deep, quadrilateral muscle spanning between the ulna and radius just proximal to the wrist. Its medial attachment arises from the distal one quarter of the anterior ulna and the adjacent interosseous membrane, then passes laterally to insert on the anterior surface of the distal radius. The ulna sits medial to the radius in anatomical position, while the carpal bones lie distal to the muscle belly and the elbow joint and humerus remain proximal in the same field. Deep location is the point. Medial viewing clarifies how closely pronator quadratus conforms to the bony contour and why it functions as a dynamic stabilizer of the distal radioulnar joint during pronation and forceful grip. Clinically, its relationship to the distal radius makes it a key landmark in volar approaches for distal radius fracture fixation, where elevation of the pronator quadratus can expose the watershed line while protecting deeper structures. Loss of the normal soft tissue plane or disruption near its radial insertion can also signal injury patterns associated with DRUJ instability.
