Description
Seen through the superficial tissues in a lateral view of the male upper limb, the pronator teres spans obliquely from the distal humerus toward the lateral surface of the mid-radius. Proximally, its humeral head arises from the medial epicondyle and common flexor origin, while the ulnar head courses from the coronoid process, creating a short muscular tunnel near the anterior elbow. Deep and medial lie the ulna and the humeroulnar joint, while the radius sits more lateral and rotates beneath the pronator as the forearm turns; posteriorly, the distal triceps brachii and its tendon align with the olecranon. Pronator teres is a reliable landmark in the cubital fossa and proximal forearm because the median nerve typically passes between its humeral and ulnar heads, and the brachial artery divides just proximal to this region into the radial and ulnar arteries. Entrapment here produces pronator syndrome, with forearm pain and median-distribution paresthesias that often worsen with resisted pronation and elbow flexion, in contrast to isolated carpal tunnel symptoms. Tightness or hypertrophy of pronator teres also affects the biomechanics of pronation-supination after radial head fracture, distal biceps repair, or prolonged immobilization.
