Description
Seen from below, the paired pubococcygeus muscles arc posteromedially from the posterior body of the pubis and adjacent tendinous arch, meeting their counterparts to form a medial U-shaped sling across the pelvic outlet. The fibers lie superior to the perineal membrane and sweep toward the anococcygeal raphe and coccyx, blending laterally with adjacent levator ani components along the inner surface of the obturator internus fascia. Bony landmarks framing the field include the pubic symphysis anteriorly, the ischiopubic rami and ischial tuberosities laterally, and the sacrum and coccyx posteriorly. Fiber direction is clear. This inferior perspective is the one you want when teaching how the levator ani closes the urogenital hiatus and supports the rectum, prostate, and bladder neck in the male pelvis. It also maps directly onto operative and procedural anatomy: the pubococcygeus and puborectalis sling form the muscular boundary encountered during perineal approaches to the prostate and urethra, and their relationship to the anal canal matters when planning fistula surgery or interpreting levator ani avulsion patterns seen on pelvic floor ultrasound and MRI. Pelvic floor hypertonicity and chronic pelvic pain often localize tenderness along these medial fibers.
